If your student is NOT planning to attend Montgomery Elementary School next school year, please complete the attached form and provide as much information as possible. Return the form to Mrs. Faia at [email protected].
If your student attended Montgomery this year on a Transfer of Attendance, you must submit another Transfer of Attendance form to the Educational Services Center for the 2024-2025 school year.
